BILE DUCT INJURY DURING CHOLECYSTECTOMY: DIAGNOSIS AND TREATMENT

Abstract
The results of re-interventions in 68 patients with intraoperative extra-hepatic bile duct (EHBD) injuries during cholecystectomy are presented. 46 patients (68,5%) were women, 22 (31,5%) were men. Patients’ age was 52.4 years on average. In 20 patients section and suturing of common bile duct at the cystic duct level, in 16 – parietal (9) or full (7) ligation, in 18 – clipping at the level of the cystic duct, and 14 cases of common bile duct defects are identified. Endobiliary intervention (EBI) applied in 34 patients (50,0%) as a first stage surgical treatment of intraoperative EHBD injuries with simptoms of mechanical jaundice (MJ, 21), purulent cholangitis (PC, 10) and external biliary fistula (3). Restorative surgery was performed in 22 patients (32,4%), reconstructive surgery – in 46 patients (67,4%). 4 patients (5.9%) died postoperatively. The cause of death was anastomosis and suture failure, persistent peritonitis (2), renal failure accompanied by severe intoxication with purulent cholangitis and sepsis (1) and pulmonary embolism (1)
