A CASE REPORT: SECONDARY SKIN PLASTY AFTER CLOSING DEFECTS OF THE HAND AND FINGERS WITH A RADIAL FLAP

Abstract

The article presents a case of patient with extensive lacerated-bruised wounds of dorsal skin of the right hand and with soft tissue defects of 2nd, 3rd, 4th and 5th fingers, with damage of the tendons of extensors of 2nd, 3rd and 4th fingers, open fractures of the heads of 2nd and 3rd metacarpal bones and the main phalanges of the 2nd and 4th fingers, with decompensation of the blood circulation of the back skin flap, after 8 hours after injury, the tendons of the extensors of the 2nd, 3rd and 4th fingers were restored, formation of artificial syndactyly between 2nd, 3rd and 4th fingers and closure of the defect area of the back surface of the hand and fingers with transposed radial flap on reversed blood flow was made. After 5 months 2nd stage of plasty was performed: the separation of artificial syndactyly between the 2nd, 3rd and 4th fingers of the right hand with the formation of interdigital spaces by moved triangular flaps on the pedicles cut out from the distal edge of the radial flap, without using free auto cutaneous grafts with a good functional and cosmetic effect.
