Our experience in the use of the Ilizarov’s apparatus in its various configurations for combined and multiple injuries of the skeletal system

Abstract

Aim. To study the influence of the quality of stabilization of two-ring, multi-ring configurations of devices and
traditional methods (plaster immobilization, skeletal traction) for the primary stabilization of fragments of
shin bones in patients with combined and multiple trauma.
Material and methods. The results of treatment of 43 patients with fractures of long bones with combined and
multiple injuries, who were treated at the Navoi branch of the RRCEM were studied. The first group consisted
of 22 (51.1%) patients with a two-ring and multi-ring arrangement of the device, the second group included 21
(48.9%) patients who were treated with traditional methods (plaster immobilization, skeletal traction).
Results. The analysis of the frequency of early complications in patients of group 2 revealed significantly
distinguishable complications depending on the type of fixation of the injured segment using different methods
of functional fixation (plaster immobilization, skeletal traction). When using plaster castings and skeletal
traction at the first stage of treatment, the risk of early complications increased distinctly, which indicated
insufficient fixation and difficulty in caring for patients. The best results were noted at the use of external
fixation devices.
Conclusion. The use of the Ilizarov apparatus at the first stage of treatment for stable fixation in temporary
osteosynthesis and after remounting in some patients for final osteosynthesis allowed to obtain reliably
positive results.
